Individualized Case Management

Upon admission to LifeHouse Group, each resident undergoes a risk and needs assessment to determine the types of services and opportunities that will best suit that individual. Residents are assigned to a case manager who meets with them regularly to help them determine how to rebuild their lives. Case managers are responsible for developing individualized recovery plans with each resident based on their personal goals, including developing job and life skills, creating educational goals, and participating in financial literacy classes. Our case managers help residents develop plans for personal and professional transformation as determined by the individual’s needs. They assist with a spectrum of issues, ranging from obtaining appropriate identification and opening bank accounts to addressing mental health issues or personal challenges with overcoming addiction. LifeHouse recruits its case managers from outside the organization as well as from program alumni who have demonstrated the ability to succeed in their recovery and guide others who are intent on changing their behavior to change their lives.
